About the role

CORE RESPONSIBILITIES:

1. Deliver case management to achieve client outcomes

a. Comprehensive, trauma-informed, strength-based and person-centred Assessment

b. Manage safety through thorough risk assessment and safety planning with clients and relevant support people to address DFV and child safety concerns

c. Conduct ‘Home Safety Audits’ and coordinate facility up-grades in client’s homes

d. Conduct holistic, client-led and culturally safe case planning

e. Provide direct support through information, emotional and material support related to client’s needs and building strengths

f. Build and maintain positive working relationships with external agencies to provide streamline and effective referrals and client outcomes

g. Provide strength-based client advocacy and case coordination internally and externally

h. Conduct formal and informal client reviews to evaluate strengths, progress and emerging needs and goals

i. Contribute to measuring client outcomes and service delivery continuous improvement though implementing client outcome measures, collecting client feedback and complaints, and contributing to changes as appropriate to role and delegation

j. Produce high quality client record-keeping including case notes, data and documentation

k. Perform after-hours (including evenings and weekends) On-call duties as part of a rotating roster

l. Perform any other duties as requested by your manager which support WAGEC’s service delivery aims across all programs.

2. Leadership and program development

a. Participate in sector agency meetings as a representation of WAGEC and its clients such as Safety Action Meetings (SAMS), Interagency meetings

b. Provide advice, guidance and input into the development and review of the program and team

c. Contributes to the development of service policy, procedures and resources

3. Maintain personal, professional and program effectiveness

a. Demonstrates appropriate inter-personal skills like active listening, empathy, social awareness and emotional intelligence in verbal communications

b. Build and maintain sector networks and partnerships through a variety of ways

c. Maintain awareness of emerging issues and policy environment, with specific focus of intersectional DFV

d. Work collaboratively and independently to manage day-to-day capacity in line with prioritising workload based on client and team needs

e. Perform practical support and manual handling tasks including but not limited to transportation of clients and goods

f. Engage in regular Professional supervision

g. Actively participate in staff meetings and learning initiatives

h. Actively participate in One-on-one supervision with line manager

i. Maintain a self-care/ professional development plan

4. Program administration

a. Maintain accurate financial records within budget

b. Submit timesheets, leave requests, and other administration

c. Apply Policy, procedures and processes to daily work
