About the role

WAGECs Case Managers work collaboratively with women, children, and families in our Specialist Homelessness Services (SHS) and other WAGEC programs. Case managers provide trauma-informed, strengths based and client-centered case management with the aim of improving client’s safety, self-sufficiency, and healing. Case Managers are co-located at sites with clients in refuges or at our drop-in service at Redfern.

Core responsibilities

1. Deliver case management to achieve client outcomes

a. Comprehensive, trauma-informed, strength-based and person-centred Assessment

b. Manage safety through thorough assessment and safety planning with clients and relevant support people

c. Conduct holistic, client-led case planning d. Provide direct support through information, emotional and material support related to immediate and on-going needs and building strengths e. Work collaboratively with Client Services staff to identify and implement strategies that support continuity and integration of care for clients

f. Provide referrals to internal and external services to meet client needs and goals g. Provide client advocacy internally and externally

h. Facilitate and perform case co-ordination internally and externally

i. Conduct formal and informal client reviews to evaluate strengths, progress and emerging needs and goals.

j. Contribute to measuring client outcomes and service delivery continuous improvement though implementing client outcome measures, collecting client feedback and complaints, and contributing to service improvements as appropriate to role and delegation

k. Produce high quality client record-keeping including case notes, data and documentation

2. Maintain personal, professional and program effectiveness

a. Demonstrates appropriate inter-personal skills like active listening, empathy, social awareness and emotional intelligence in verbal communications

b. Maintain sector networks and partnerships

c. Maintain awareness of emerging issues and policy environment

d. Work collaboratively and independently to manage day-to-day capacity in line with prioritising workload based on client and team needs

e. Perform practical support and manual handling tasks including but not limited to transportation of clients and goods, site cleanliness

f. Engage in regular Professional supervision

g. Actively participate in staff meetings and learning initiatives Position Purpose: Case Manager

h. Actively participate in One-on-one supervision with line manager

i. Maintain a Staff Training and Development plan

3. Program administration

a. Maintain accurate financial records within budget

b. Submit timesheets, leave requests, and other administration

c. Apply Policy, procedures and processes to daily work

4. Contribute to WAGEC’s organisational culture

a. Work in a manner consistent with WAGEC’s vision, values and strategy

b. Apply WAGEC’s Ethical Stance to ethical dilemmas, managing your professional and personal obligations at work.

c. Contribute to developing a culture of safety through risk and Work Health and Safety management processes

d. Contribute to WAGEC’s overall social impact

e. Occasionally perform other duties as requested by Management
