Specialist Homelessness Services (SHS) Case Manager

Full time

THE ROLE

WAGEC's Case Managers work collaboratively with women, children and families in our Specialist Homelessness Services (SHS) and other WAGEC programs. Case Managers provide trauma-informed, strengths-based and client-centred case management with the aim of improving client’s safety, self-sufficiency, and healing.

This is a full-time, permanent role based in Burwood.

  • Deliver crisis response and case management for WAGEC clients via telephone, at WAGEC sites or via outreach services
  • Work collaboratively with WAGEC teams and external stakeholders to find pathways for support for clients
  • Conduct risk and needs assessments, assist with safety planning and referrals to other services
  • Provide occasional after-hours On-Call duties as part of a rotating roster
  • Work collaboratively to support team operations and contribute to team goals
  • Proactively and positively contribute to WAGEC’s culture and impact.

ABOUT YOU

  • You have 3-4 years of case management experience working with people in human services.
  • Significant experience and/or knowledge in domestic/family violence and/or homelessness services will be highly regarded for more specialised roles.
  • You have a high level of understanding of working with people who have complex needs
  • You understand risk management, safety planning and the importance of advocacy
  • You work from a trauma-informed, client-centred, strengths-based approach
  • You are a supportive, team-player who enjoys sharing knowledge with other workers and students
  • You’ll have Social Work, Psychology or Community Services qualifications or experience, or similar, and are committed to continuous learning.
